I. Assistance in Crisis Situation

The CITY SOCIAL WELFARE & DEVELOPMENT OFFICE (CSWD) assists its people in times of calamities and these less privilege in times of hardship

DOCUMENTARY REQUIREMENTS

    Barangay Certification with:

        1. Medical Assistance – Doctor’s prescription, Barangay Certificate

        2. Burial Assistance – Death Certificate, Barangay Certificate/ (If Veterans) – Certification from Post Commander

        3. Fire, Typhoon/Flood Victim Assistance – (If Caused by Fire/Burned)*Picture of damaged properties, Barangay Certificate/in the absence of Barangay Captain, the Barangay Kagawad In-charged of the Day shall be the one to provide the Barangay Certification.

HOW AVAIL ASSISTANCE IN CRISIS SITUATION

    1. Present documentary requirements, Fill-up intake form and sign in logbook

    2. Forward Barangay Certification together with supporting document for approval of the City Mayor

    3. Bring to City Budget Office documents with amount approved for release

    4. Determine availability of budget

    5. Return Obligation Voucher Request for Signature CSWD Officer

    6. Bring back to Budget Office for signature of Obligation Voucher Request (OVR)

    7. Processing of Documents at Accounting Office

    8. Processing of Documents at Treasurer’s Office

    9. Forward documents for signature of City Administrator

    10. Forwarded documents for disbursement of approved fund

        Note:

            a. Applicable only for cash assistance P1,000.00

            b. and below

            c. Above P1,000.00 is issued by check

            Note:Issuance of check is subject for normal process/check will be signed in Admin Office

    11. Release of Assistance amount to the Client

II. Management of Women and Children especially in Difficult Circumstances

The CITY SOCIAL WELFARE AND DEVELOPMENT OFFICE (CSWD) attends to abused women and children in Sagay City and extends necessary assistance

HOW TO AVAIL OF THE ASSISTANCE

    1. ABUSED WOMEN

        a. Interview of client and conduct counseling service

        b. Refer client to WCPD – PNP

    2. CHILD ABUSED

        a. Interview with the client/giving of information

        b. Assist client to City Health Office for Medical Examination

        c. Conduct Counseling

        4. Assist client to PNP Station for the execution of affidavit, then to the Fiscal’s Office for filling of case

        5. Assist client during court hearings.

III. Preparing Social Case Study Report

THE CITY SOCIAL WELFARE AND DEVELOPMENT OFFICE assists under previledge constituents who cannot afford medical and hospitalization expenses by preparing a Social Case Study Report to be submitted to Charitable Institutions and other concerned agencies that can give financial/medical assistance.

IV. Management of Children/Juvenile in Conflict with the Law

THE CITY SOCIAL WELFARE AND DEVELOPMENT OFFICE extend social assists to juvenile in conflict with the law in form of an intervention and divertion program

    a. Blotter Report

    b. Birth Certificate/Baptismal Certificate/School Records [Proof of minority]

    c. Medical Certificate

How to Avail of the Assistance

    15 YEARS OLD W/O DISCERNMENT AND BELOW

    a. Submit child for an interview

    b. Contact parents for release on recognizance

    c. Implement necessary intervention

    ABOVE 15 YEARS OLD BUT BELOW 18 YEARS OLD W/ DISCERNMENT

    a. Submit child for interview and counseling

    b. Contact parents for release on recognizance

    c. Home visitation/investigation

    d. Prepare Case Study Report (for court submission)

    e. Assist client in every court hearings

V. Day Care Program

THE CITY SOCIAL WELFARE AND DEVELOPMENT OFFICE assists in the provision of supplemental parental care to 0 to 6 years old children who maybe neglected, abused, exploited or abandoned, during part of the day whenthe parents cannot attend to their needs.

    Baptismal Certificate or Birth Certificate

    a. Bring choldren 3-5 years old for interview and admission

    b. Conduct regular class

    c. Issuance of Certificate of Completion

VI. Progam for Mental Clients

THE CITY SOCIAL WELFARE AND DEVELOPMENT OFFICE assists residents with mental disorder in their psychiatric treatment.

DOCUMENTARY REQUIREMENTS

    a.Referral letter from Brgy. to DSWD

    b. Intake form of Local Supervisor [BHW]

How to Avail of the Service

    a. Parents/Guardian submit complete requirements and interview

    b. Conduct to NORFI every Monday [4 clients/week]

    c. Evaluate client status

    P300.00 medicine/good for one month

VII. Program for Persons with Disabilities

THE CITY SOCIAL WELFARE AND DEVELOPMENT OFFICE assists residents of Sagay City with physical disabilities in terms of their medical & other needs.

How to Avail of the Service

    a. Submit/file application form

    b. Interview the client by Focal person

    c. Prepare the I.D. and facilitates signature of CSWDO/Mayor

    d. Release of ID and availment of services

VIII. Solo Parent Program

THE CITY SOCIAL WELFARE AND DEVELOPMENT OFFICE assists solo parents in the issuance of their identification cards to avail of benefits from different government agencies under R.A 8972

    2. Death Certificate – Birth Certificate of minor children

    4. Residence Certificate

IX. Señior Citizens I.D. Services

THE CITY SOCIAL WELFARE AND DEVELOPMENT OFFICE assists elderly in the issuance of their identification cars to avail of benefits under RA 9257.

    2. Photocopy of Birth Certificate

X. Medicare Para Sa Masa Program

THE CITY SOCIAL WELFARE AND DEVELOPMENT OFFICE through its SCALA assist the indegent families of their membership cards to avail of their benefits for free hospitalization expenses in PHILHEALTH accredited hospitals under RA 9241

XI. Responsible Parenthood Movement Class

THE CITY SOCIAL WELFARE AND DEVELOPMENT OFFICE assist couples in trouble/problem through holding class, counseling and home visitation.

How to Avail of the Service

    a. Consult the social worker of your problem

    b. Conduct personal interview of client

    c. Visit/Interview of family members,validation of information

    d. Conduct classes/counseling to couples/monitoring

XII. Pre-Marriage Counseling

THE CITY SOCIAL WELFARE AND DEVELOPMENT OFFICE assist would be couples applying for marriage through counseling Re: Their duties and responsibilities as future parents to their children.

  • Clientele: Clients needing supporting  papers for  hospital, PCSO,NBI, National Kidney Institute, Phil. Heart Center for  Asia, Courts and other agencies for referral.(Note:Only the client or the immediate member of the family is allowed to request for the case study.)
  • Requirements :
    • certification as an indigent resident from your barangay
    • medical abstract for medical-related cases to Social Worker and concern client be available for interview. Home-visit is optional.
